SLC Registration Correction/Addendum Form

This page is used to submit registrations for the following reasons:

  • Name Corrections
  • Adding Additional Contest OR Previously Registered Student Adds Contest as Regional Contest Alternate  
  • Alternates/Substitutions  
  • Additions OR New Student Becomes Eligible As Regional Contest Alternate

Name Corrections

If you need to correct the spelling of a conference participant’s name, please email the state office at info@michiganbpa.org to notify them of the change. Please put in the subject line “SLC Name Spelling Correction” and indicate the following in the body of the email: 1) the current spelling, 2) the correct spelling, 3) contestant number (if it is a student), and 4) school name. No additional payment is necessary for name corrections.

Adding Additional Contest OR Previously Registered Student Adds Contest As Regional Contest Alternate

If a student is already registered for the conference via the BPA Scheduler website and he/she is adding an additional contest,  prepare a Registration Correction/Addendum Form (see below) with all the appropriate information listing all the information for the student who is adding an additional event. Check the “Adding Additional Contest” box and enter the event name of the competition the student is adding.

NOTE: If the student was not already registered for the conference, a Registration Correction/Addendum Form and Housing Change Form must be completed. Original registration rates will apply.

Alternates/Substitutions

These are the instructions a chapter must follow in order to register a student/advisor/chaperone for the conference if the individual is an alternate/substitute and the chapter registration materials have already been submitted. This procedure applies to changing one person’s name to another’s on the SLC chapter registration.

Original registration fees will apply. Late fees will not be charged if the change is made more than ten (10) calendar days before the first day of the conference. If the change is made less than ten (10) calendar days before the first day of the conference, the difference between the advance registration rate and late registration rate will be charged for each instance.

Students who are being added because they are an alternate from regional competition who just became eligible to compete, will be charged original registration fees regardless of the date they are added.

SLC Registration

Prepare a Registration Correction/Addendum Form (see below) with all the appropriate information listing all the information for the alternate/substitute person(s) only. Do not resubmit names that were previously submitted. Also, indicate the name of the person that is dropping. Partial refunds via a Registration Refund Request Form are not allowed if an alternate/substitute’s name is being submitted.

HOTEL RESERVATION

  1. Prepare a Housing Change Form online at http://www.michiganbpa.org/slc-housing-change indicating any adjustments to any pre-existing rooms that you have reserved. If you require a completely new room, mark the “New/Additional Room” box.
  2. If you have already received notice of what hotel you have been assigned to, make sure you indicate what hotel you are assigned when you complete the form.
  3. Do not submit any form of payment. Your hotel bill will be adjusted accordingly automatically.

Additions OR New Student Becomes Eligible as Regional Contest Alternate

These are the instructions a chapter must follow in order to register a student/advisor/chaperone for the conference if the individual is being added and the chapter registration materials have already been submitted via BPA Scheduler.

If the additional student is a regional contest alternate, regular registration fees will apply. All others, late registration fees will apply.

SLC Registration

Prepare a Registration Correction/Addendum Form with all the appropriate information listing all the information for the additional person(s). DO NOT resubmit names that were previously submitted.
